Piranhado inDiablo 4is a throwback to the Witch Doctor Piranhas rune of the same name from the previous game. It returns as a Witchcraft Power in Season 7, one that players can have active regardless of what class they play.
Having what is basically a character class' entire skill as a passive is neat and all, but getting Piranhado won’t be easy. Like with the rest of theLost Witchcraft Powers inDiablo 4, getting this one is going to take time, energy, and a lot of luck.

How Do You Unlock Piranhado in Diablo 4?
Piranhado is one of the four Lost Witchcraft Powers, and to find it, you need tofind a Forgotten Altar. These altarsappear randomly in dungeons, whether they’re normal ones, Nightmare Dungeons, or maps in the Pit of Artifficers.
Piranhado spawns a monster-seeking, piranha-filled tornado that sucks monsters into it. When equipped, Piranhados will spawn when an enemy is affected by both a Hex and an Aura effect.

Forgotten Altars onlyspawn on Torment difficulty, and their spawn rates increase according to your currently-selected Torment level, starting at 1% and capping out at 5% as of Patch 2.1.0 (this may increase later in the season). You can increase your odds of finding a Forgotten Altar for a limited time by using a Draught of Whispers, a consumable item that doubles Forgotten Altar spawn rates while also increasing the likelihood of finding a Fugitive Head while on Headhunt events.

Best Way to Find Forgotten Altars
Due to the random nature of Forgotten Altar spawns, the best places to search for them are dungeons with small layouts. One of the best dungeons for this isHoarfrost Demisein the Fractured Peaks. You can find this just north of the Bear Tribe Refuge Waypoint, which is northeast of Kyovashad.
Hoarfrost Demise’s layout is very small compared to most dungeons, and it’s mostly composed of short corridors that circle in on each other, forming tight loops that are easy to clear and navigate. Mobile builds likeCentipede Spiritborn inDiablo 4will have a very easy time searching Hoarfrost Demise.
If you’d rather spend your time more efficiently, then we recommend running the Pit instead. You’ll likely have a smaller chance of finding a Forgotten Altar here due to the larger levels, but at the very least, you’ll finish each run with the chance to upgrade your Glyphs.
