Hand of Wisdom and Actionis a Unique Glove that is a possible drop for defeating Xesht, We That Are One, the endgame pinnacle Breach boss. Xesht can be encountered inside the Twisted Domain, which can be accessed via the Realmgate on the Atlas by using a Breachstone.

HoWA can also be obtained on thePath of Exile 2official trading website, though this can be very expensive as even the worst rolled HoWA can go for multiple divines.

No, you cannot. Hand of Wisdom and Action is a limited drop from Xesht, We That Are One, and is not part of the regular loot table, unlike Astramentis,Headhunter, orPolcirkeln. This means you cannot use anOrb of Chanceon White Furtive Wraps to take a gamble on possibly acquiring it.
